After the referral, our Community Life Planning Coordinator will arrange a visit with you to determine what kind of support we can offer and provide information of services available in the community.

Initial Assessment

Once we’ve identified your needs, we will work with you to set up an achievable Life Plan. This plan can involve aspects such as connecting you with community activities/groups, helping you seek support with your care needs or caring role, befriending services, access to other support near the Hoo Peninsula, etc.

Whilst wHoo Cares offers a range of activities and support, there are also other services we can help you to get access to, both within and outside the Peninsula.

Connecting with services

Once identified together with your Community Life Planning Coordinator during your assessment, our volunteers’ support would be available to you. Our fantastic volunteers could help with befriending, transport, support in the community, etc.

Help from our Volunteers

The support that you need might change with time, and wHoo Cares will be with you every step of the way to find solutions. Our service doesn’t stop after a period of time, support will be offered continuously whilst needed.
